Are you really sure there is sustained fire bonus up to (+3)? I usually don't use the log, but once after MANY misses in a row I turn the log on to see the proof of the "incredible impropability" and what I saw - there were also 3-rolls after 4th shot, but no hit at all. If the 4th (and the following) shot(s) shall get (+3) bonus, then 3-roll equals 6 total, so it should be hit, am I right? So what's wrong? Any suggestions?
What enemy was you firing at?
I can't say I am 100% sure everything works but at least 99% since we have tested it a lot and double checked the log.

One possibility is if you selected another marine any time in the middle, the bonus could reset to start. Even if you didn't make any actions with the other marine.
I know walk and fire shooting works that way. In that case, you lose the 0 AP shot if you select another marine.

It was about 6 in-row shots (no move, no switch). By the way, I'm quite sure I didn't lost the sustained fire bonus even when I switch to another marine in the other case. Maybe I'll start watching logs regurarly ...

I believe the log shows the roll with the SF bonus already applied.

No it doesn't. There is not any "SF bonus" in the game, there is just lower roll-condition for a successful hit, so you can roll the exactly same roll three times, then log shows
roll (1, 4) 4
roll (1, 4) 4
roll (1, 4) 4
but only the 3rd shot will be a hit.
Just checked the code and you are correct. Sustain fire bonus will never be larger than 2 so a roll of "3" (3+2 < 6) will never kill an alien.
You will loose sustained fire bonus:
when performing an action that is not firing your weapon (full auto is not counted as firing your weapon)
when ending your round
when activating overwatch
when shooting at another target
when you run out of ammo
when your weapon jams
You will not loose sustained fire bonus if you switch to another soldier, however you will loose any chance of free fire after moving.
